  2. January 22, 2009 - Response: CA Government Code Section 6253(9)(b)(1) states that public agencies do not have to reconstruct a record if the agency no longer has the record. The Department has not had and does not maintain a list of the names and descriptions of reports using information stored in Chameleon. A list of Chameleon reports available on line was provided instead. Department cites [Goverment Code Section 6253.9(b)(1)]

  2. January 27, 2009 - Response(via USPS): The request requies IT division to create a specific SQL query to compile records in electronic format and then export to Excel. A charge is permitted under Govt. Code Sec. 6253.9(b)(2) for creating the compilation. Estimate is that it will take two (2) hours to create this electronic record at $32.00 per hour. Please send a check for $64.00 so that we can create this record for you.
  3. February 2, 2009 - Reply(via Email): To reduce the length of time IT division would need to satisfy this request for Chameleon records, have any SQL queries been previously created and used to extract animal records from Chameleon that would result in more data fields than necessary but still contain at a minimum, the data fields being requested?.
  4. February 12, 2009 - Response: Records available upon receipt of check for $32.
  5. February 17, 2009 - Reply: Payment of $32 made payable to the County of Los Angeles.
  6. February 23, 2009 - Response: Records received on CD via USMail on 2-24-2009.

  2. January 29, 2009 - Response(via USPS): Copies of Kennel Impound Card and medical history for impound number A925098 provided.
  3. February 2, 2009 - Reply(via Email): Response is missing requested information (i.e. Outcome Date, Outcome Type, etc..) and medical history shows treatment was prescribed but no treatment cards/information was provided to verify the animal received treatment. Please provide any and all records and information as originally requested.
  4. February 12, 2009 - Response(via Email): Outcome information provided in the form of text written into an email and Memo information not complete.
  5. February 18, 2009 - Reply(via Email): Request time/date of each memo entered and Memo Type for each be provided.
  6. April 1, 2009 - Update: Department Failed to Respond in the time allowed by law and Failed to Provide the requested information. [Government Code Section 6253.(c)]

  3. February 12, 2009 - Response(via Email): No reports or documents responsive to this request exist.

    COMMENTS: Policies and Procedures Vol-2 Section-309 The Department Forms Control Officer (DFCO) is required to conduct a Department survey at least once every fiscal year to determine whether any forms have become outdated or obsolete and then prepare a report for the General Manager's review of any outdated or obsolete forms along with DFCO recommendations. Pamphlets, flyers and booklets are to be included. Based on the Department's response, there have been no outdated or obsolete forms, pamphlets, flyers or booklets in the past two years..

  1. January 26, 2009 - Submit PRA via Email: Ross Pool cc Ed Boks, Linda Barth
  2. February 15, 2009 - Update: Department Failed to Respond in the time allowed by law and Failed to Provide the requested information. [Government Code Section 6253.(c)]
  3. March 2, 2009 - Response(via Email): Department provided a total of 120,430 animal records in a tab-separated text file for Jan07-Dec08 impounds. DOB (date of birth) was provided instead of Age (years/months) because the year and month is not used as consistently. Tag Type and Tag Number information was not provided. Department does not give out this information.
  4. March 2, 2009 - Reply(via Email): Request that Department confirm numbers (1-15) being entered for Jurisdiction represents City Council Districts. Request that the exact reasons be given for not providing Tag Type and Tag Number and to provide the name and title of the person who made this decision. [Government Code § 6253.(c)(d) § 6255.(a)(b)]
  5. March 5, 2009 - Response(via Email): Confirmation that the numbers (1-15) entered for Jurisdiction represent Council Districts. The Department never gives out tag number because this number ties back to an individual’s address and is considered personal and private information.
  6. March 5, 2009 - Reply(via Email): Provide the name and title of the person who denied the request for Tag Type and Tag number based on this determination. [Government Code § 6253.(d)]
  7. March 5, 2009 - Response(via Email): Decision was made by Linda Barth, Assistant General Manager in consultation with Dara Ball of the Systems Group.
FAIL  KCAC - PR065: Chameleon records of all animal impounds from Jan07 - Feb09
  1. April 1, 2009 - Submit PRA via Email - Note: While this request may appear to be a duplication of previous requests, it is not. Additional Chameleon data fields are included in this request.
  2. April 6, 2009 - Response(via Email): Department does not have DOB on 99% of the intakes. Will rerun query.
  3. April 6, 2009 - Resply(via Email): Please re-run query and include both DOB and age in Years/Months.
  4. July 6, 2009 - Update: Department Failed to Respond in the time allowed by law and Failed to Provide the requested information. [Government Code Section 6253.(c)]

  2. September 28, 2009 - Request confirmation from Ross Pool that PRA request was received. No response.
  3. October 1, 2009 - Request confirmation from Ross Pool that PRA request was received.

  4. October 1, 2009 - Response: Ross Pool responds and Department needs the 14 day extension. [Government Code Section 6253.(c)]
  5. October 13, 2009 - Response: Ross Pool sent animal records via email; copied Dara Ball and Linda Barth. As with PR064(above), Department failed to provide Tag Type and Tag Number information as requested, failed to provide the exact reason for not providing this information and failed to provide the name and title of the person who made this decision [Government Code § 6253.(c)(d) § 6255.(a)(b)].

    Animal Records were not provided in any of the industry standard file formats as requested. Duplicate Chameleon field names were found. Additional processing was required before records the Department provided could be used.

  1. September 28, 2009 - Submit PRA via Email: Dr. Eric Anderson Due: 10-08-09
  2. October 7, 2009 - Response From Dr. Anderson: A new 'program'(query) needs to be created to retrieve the addtional data fields at a cost of $54.43 OR for a processing fee of only $15 a previously retained report (with limited information) can be generated. Which would I prefer?
  3. October 8, 2009 - Reply To Dr. Anderson: Please have Animal Service IT staff create the new 'program'(query) with additional data fields and when complete send a sample of one or two records to verify staff has included all data fields requested and can produce records in one of the electronic formats specified.
  4. October 10, 2009 - Response: Dr. Anderson forwarded request to IT staff along with directions that report format be retained for future requests.
  5. October 13, 2009 - Reply:Request estimated time to expect animal records.
  6. October 13, 2009 - Response: Dr. Anderson provided a detailed description of how SLO uses some of the Chameleon data fields, wanted clarification on whether I wanted some addtional data fields and anticipates records could be provided within the next few days.
  7. October 13, 2009 - Reply: Asked to please include additional data fields. Asked to please expedite the processing of this request if possible and explained why.
  8. October 14, 2009 - Response: Dr. Anderson provided animal records in MS Excel format along with notes from himself and Kermit Spears, the Department's Automation Specialist II who did the work and retrieved animal records.

    Thanked them for expediting the request to meet deadline.
