  7. February 24, 2009 - Reply: Returning invoice and a payment of $25 for records with the understanding that these records are copies of actual euthanasia logs for Santa Cruz and Watsonville and will be provided electronically as Adobe PDF files.
  8. March 20, 2009 - Response: Euthanasia records provided in two MSExcel files however the request was for copies of the actual euthanasia logs and were to be provided electronically as Adobe PDF files.
  9. March 22, 2009 - Reply: Requesting that copies of the actual euthanasia logs for Watsonville and Santa Cruz shelters be provided as originally requested. Copies of PR055 and subsequent correspondence attached.
  10. April 8, 2009 - Response(via USMail): Paper copies of euthanasia logs received in two separate mailing packets. Approximately 2" of the right side has been removed(cut) from each page. It is possible this section of each page may have contained information related to the person performing euthanasia. Reason for not providing complete records and the names and titles of persons responsible for this decision were not provided. [Government Code § 6253.(c)(d) § 6255.(a)(b)

  4. February 3, 2009 - Response: Records of ALL intakes provided and data field Jurisdiction included. Crystal Reports was reported to be problematic when attempting to include the additional data fields.
